Optical Scan

Optical scan answer sheets or test booklets are used when responses to paper-and-pencil test forms are scored with optical scanning software. Optical scan answer sheets require responses to be entered with answer bubbles. For this reason, suitable items are those that use online radio buttons and most other selected response item types.

Single- or multiple-select items and matrix items are available in an optical scan format.
